(I'll send it to you the day before our appointment so you can read it calmly and without pressure, and I'll answer all your questions during the appointment).
What is the purpose of this document MANDATORY ?
- To inform you about contraindications, care, touch-ups, terms and conditions…
- Protecting each other by describing each person's commitment to this project.
- To realize that getting a tattoo is not a trivial matter.
You promise me:
- Be of legal age.
- Having been informed of the care to be followed during the healing period.
- Having been informed of the potential risks involved.
- Inform of any allergic reaction or any known or suspected contraindication before the tattooing procedure.
- Not to be under the influence of any legal or illegal substance that could impair your judgment, and that the work carried out by Hyper Ma is done with your full consent.
- Not to be pregnant.
- To release the tattoo artist from all consequences due to the tattoo and to assume full responsibility for it.
- Authorize Hyper Ma and the shop to use photos of your tattoos for professional purposes.
- Authorize Hyper Ma to process the data collected in order to contact you and send you information related to its business.
- And attests on my honor that I agree to be tattooed by Hyper Ma.
I promise you in return:
- To provide you with the best possible information about the act of tattooing and everything that follows from it (care, procedure, risks…)
- To gently anchor WHAT YOU WANT.
- To ensure you have a great time so that you leave with a huge smile, in addition to the tattoo.
